Opening Keynote: Transforming Care Delivery with Hospital at Home
Bruce Leff, MD, Professor of Medicine, Director, Center for Transformative Geriatric Research, Johns Hopkins University School of Medicine
Hospital at Home (HaH) provides acute hospital-level care at home in lieu of traditional hospital care. Over several decades, HaH has been shown to provide better care at lower costs than traditional care. Implementation of HaH in the U.S. has grown exponentially in the wake of the CMS Acute Hospital Care at Home payment waiver. In this session, Dr. Bruce Leff will discuss the development and evidence base for HaH, the dissemination of the model into US healthcare and the potential for HaH to spark the development of a fully-baked home-based care ecosystem.
